Real-time Ultrasound Guided Epidural Catheter Placement in Morbidly Obese Parturients after Unsuccessful Conventional Attempts

چکیده
Several studies have emphasized the usefulness of preprocedural Ultrasound (US) assessment of lumbar spine. Despite those helpful informations this procedure may remain challenging. This report evaluates the effectiveness of a Real Time US Guided Epidural Anesthesia (RTUGEA) in morbidly obese patients as usual loss of resistance technique combined with a preinsertion US assessment of lumbar spine had failed. We performed successfully RTUGEA, in ten morbidly obese parturients. The epidural space was identified in all patients and one to three attempts was necessary to reach the epidural space. The tip of Tuohy needle was not visible in 4 cases. We were able to identify the catheter in one out of ten cases. None inadvertent dural puncture was recorded. Recovery from epidural analgesia was uneventful in all cases. To our knowledge, there is no published data on real time ultrasound guided epidural anesthesia in morbidly obese patients. Our data support the usefulness in morbidly obese patients of a RTUGEA for lumbar access. Further extended and randomized study is needed to assess safety and efficacy of such procedure. Epidural Catheter Placement in Morbidly Obese Parturients with the Use of an Epidural Depth Equation prior to Ultrasound Visualization
BACKGROUND Previously, Balki determined the Pearson correlation coefficient with the use of ultrasound (US) was 0.85 in morbidly obese parturients. We aimed to determine if the use of the epidural depth equation (EDE) in conjunction with US can provide better clinical correlation in estimating the distance from the skin to the epidural space in morbidly obese parturients. METHODS One hundred ...

متن کاملObesity and obstetric anaesthesia.
The prevalence of obesity continues to increase despite preventive strategies. Obese parturients are at increased risk of having either concurrent medical problems or superimposed antenatal diseases such as pre-eclampsia and gestational diabetes. Moreover, they have a tendency to labour abnormally contributing to increased instrumental delivery and Caesarean section. Obesity is a risk factor fo...
متن کاملPredictive model for the inadequate labor epidural analgesia: an outcome of the prospective observational study at university women's hospital.
BACKGROUND Continuous epidural analgesic infusions provide superior analgesia as compared to other forms of labor analgesia. However, inadequate analgesia after labor epidurals is not uncommon and has been found to be as high as 24% in some studies. The mechanism of these failures include inappropriate epidural catheter location, tissue compartmentalization within epidural space, delayed migrat...

BACKGROUND We report the first prospective sonoanatomic study in neonates with the aim to perform ultrasonographic-guided epidural catheter placement in this age group. METHOD One hundred forty-five neonates with a body weight < or =4 kg (0.53-4 kg) were included in this prospective study. The study was divided into 3 consecutive parts. In the first part, the neuraxial sonoanatomy of 60 neona...
